We also used the system dynamics model to establish a PHC-based integrated health system as an alternative option for addressing hospital overwhelm. The following three critical policy interventions were tested: first contact in PHC, identification of high-risk patients and recommendation to secondary care hospitals, and referral for a return to PHC for follow-up and recovery at the community level (Figure 1).
