Evaluation of the Tu’Washindi Na PrEP Intervention to Reduce Gender-Based Violence and Increase Preexposure Prophylaxis Uptake and Adherence Among Kenyan Adolescent Girls and Young Women: Protocol for a Cluster Randomized Controlled Trial

Individuals who do not meet the criteria for participation in this trial (ie, screen failure) because of criteria that are likely to change over time may be rescreened. Examples include not being resident of the ward or not being interested in taking Pr EP. Recruitment occurs simultaneously in each pair of wards before moving on to the next pair and focuses on the catchment areas of 2 to 4 health facilities in each ward.

Older users or those less familiar with gamification might find such features irrelevant or even patronizing if they do not see clear value in them. Other gamification elements such as progress bars, interactive storylines, or more useful rewards may be more effective in motivating users. Therefore, future iterations could explore the inclusion of a simple setup and customization menu to enable or disable features such as gamification and other user-preferred functionalities.

Pre-exposure prophylaxis (Pr EP) describes a medication that is proven to be 99% effective when taken as prescribed to reduce the risk of getting HIV by taking a pill or injection among people who do not have HIV but may be vulnerable to being introduced to or “exposed” to it. The marginal use of Pr EP hinders progress toward decreasing the incidence of HIV cases among Black cisgender women.

The daily reflection, based on Acceptance and Commitment Therapy principles known for their efficacy in chronic pain management [30,31], asks users, “What did you do that mattered to you today?” Users then rate their day on a visual analog scale from 0 (Nothing) to 10 (Everything I wanted) and can record any meaningful activities. Following the daily reflection, users are invited to complete a pain record where they assess their current pain level by responding to “How is your pain right now?”
